Request for ACT 48/45 Course Reporting
In compliance with the Pennsylvania Department of Education (PDE), 吃瓜tv will report successful completion of courses taken to satisfy ACT 48/ACT 45 regulations. The report will be sent to PDE and the professional educator (the student). Records of completion of one to six credits at 吃瓜tv will become part of the student's permanent transcript. To request that your course completion (1-6 credits) be reported in compliance with ACT 48/ACT 45, you must complete this form.
